College Cellular Telephone Usage

All cell phone equipment purchases and service must be approved through a purchase order. Employees are not permitted to sign agreements, purchase services or equipment or make changes to cell phone plans.

Employees who are required to carry cell phones may select from one of two options for cell phone use. An employee may elect to use the phone for college business only or they may elect to use the phone for college business and personal use.

If the employee elects to use the phone for college business and personal business, the cost of the phone and the monthly charge will be considered as taxable income to the employee. Any downloads to the phone will be reimbursed to the college regardless of which option the employee has selected.
I understand and I agree that I am personally responsible for the equipment that has been checked out to me by Pratt Community College. The equipment is to be used for College business purposes only. I agree to accept personal financial responsibility for the equipment if lost, stolen, or damaged due to my negligence. Ordinary wear and tear is expected. I also agree to accept personal financial responsibility for any charges resulting from personal use.

Each employee with a cell phone must complete an Employee Declaration of College Issued Cell Phone Usage as shown below:

______ I will not be using the college issued cell phone for personal usage. In the event of personal usage, I will reimburse the college for the personal usage during the monthly billing cycle.

_____ I will be using the college issued cell phone for business and personal use. I understand that the total monthly charges to my cell phone number will be considered taxable income to me.
