Use this form to request that an official transcript be sent to the recipient of your choice.  Please print form, fill out requested information, and return by mail to:

Beginning Sept. 1, 2011, transcript fees will increase to $10 per transcript, and official transcripts will no longer be free to current students. Cash, check, money order or credit cards are accepted. When using a credit card, please include name as it appears on the card, address of the card holder, card number and expiration date on the form.
